肺癌

    从2000年12月至2003年12月，笔者以自拟扶正抗癌汤治疗晚期肺癌65例，疗效满意，现报道如下。
    1  临床资料：
    本组65例，均经X线胸片、CT、病理或脱落细胞学确认。其中男46例，女19例；年龄最小14岁，最大80岁，平均61岁。小细胞肺癌(SCLC)7例，非小细胞肺癌(NSCLC)58例。其中鳞癌23例，腺癌35例。卡氏生活质量评分均在60-70分
。
    2  治疗方法：
　　自拟中药扶正抗癌汤，药用：生黄芪30g，生白术10g，炒黄芩10g，七叶一枝花15g，天龙6g，白花蛇舌草、紫丹参各30g，瓜蒌壳、茯苓、富锗紫灵芝各15g，炙鸡内金10g，炒山楂、炒神曲各15g。 每日1剂水煎服。日服2－3次。3个月为1疗程。咳嗽痰多加桑白皮、鱼腥草、浙贝等；咳痰带血加白芨、三七、仙鹤草、藕节炭等；胸闷胸痛，加枳壳、郁金、延索胡等；发热不退加丹皮，生石膏、紫草、水牛角粉等；胸腔积液加葶苈子、泽泻、车前子、猪苓等；阴虚加南北沙参、天花粉、天麦冬等；阳虚加巴戟天、肉苁蓉、补骨脂等。
    3 疗效判定标准与结果：
　　按照WHO实体瘤的近期疗效评定标准，完全缓解(CR)：无；部分缓解(PR)轻度缓解( MR)36例(55.39%)；稳定(SD)4例(6.15%)；进展(PD)(1.54%)。
    治疗前后主要症状变化(咳嗽、痰血、胸痛、发热等)，显效：治疗后症状明显减轻或消失35例；有效：症状减轻21例；无效：无明显改变或加重9例。总有效率为86.15%
。   卡氏评分：治疗后90-80分25例(38%)，70-60分21例(33%)，治疗保持和提高者占71%。
    4 体会：
    肺癌，是我国目前发病率高，致死率居前列的恶性肿瘤之一。晚期病例化疗、放疗效果都不理想，且伴有明显毒副反应。祖国医学认为，“正气扶正抗癌汤，方中重用生黄芪，取其益气解毒之功；配生白术、生苡仁、茯苓可益气解毒、清利湿毒；富锗紫灵芝扶助补虚白花蛇舌草、七叶一枝花、黄芩、天龙、山慈菇清热解毒、软坚消肿以攻逐邪毒丹参活血散瘀；杏仁、半夏、瓜蒌壳宽中理气，止咳化痰；炙鸡内金、炒楂曲消导助运，保护脾胃功能。综观上方，可扶正袪邪、解毒消肿。近些年报道，以上中药中好多含有抗癌的有效成分，如山慈菇主要成分秋水碱；苡仁中含有薏苡仁酯，都是具有抗癌抑癌作用，并有提高免疫力功能作用。
      综合上述：说明扶正抗癌汤具有扶正补虚，提高机体免疫功能，抗癌抑瘤，能提高肺癌患者生存质量，改善症状，稳定病灶，延长患者生存期作用。

Day 1 Shuijianbi. Day clothes 2-3 times. 3 months of a course of treatment.      3 determine the efficacy of standards and results:
Solid tumors according to WHO standards for assessment of short-term effect, complete remission (CR): nil; partial remission (PR) with mild to alleviate (MR) 36 cases (55.39%); stable (SD) 4 cases (6.15%); Progress (PD ) (1.54%).
     The main symptoms before and after treatment change (cough, sputum blood, chest pain, fever, etc.), markedly effective: After treatment, symptoms were relieved or disappeared 35 cases; effective: symptoms were relieved in 21 cases; invalid: no significant change or increased in 9 cases. The total effective rate 86.15%
. Karnofsky score: 90-80 hours after treatment in 25 cases (38%) ,70-60 minutes in 21 cases (33%), treatment to maintain and enhance accounted for 71%.
     4 Experience:
     Lung cancer, is China's current high incidence of the cancer mortality rates among the top one. Late cases of chemotherapy and radiotherapy is not satisfactory, and accompanied by significant toxicity.
