生物治疗简介

  1. CIK细胞治疗

  CIK细胞是将人外周血单个核细胞在体外用多种细胞因子共同培养一段时间后获得的一群异质细胞,因其兼具T淋巴细胞强大的抗瘤活性和自然杀伤(NK)细胞的广谱的杀瘤优点,被认为是新一代肿瘤过继性细胞免疫治疗的首选方案。常规程序如下:细胞采集→体外诱导、分化→全程质量控制→生物学活性测定→CIK细胞回输→随访。从每疗程(细胞采集到回输结束)共10-14天,每疗程回输2次(连续两天)。该中心在2000年开展了CIK细胞治疗,涉及肿瘤包括淋巴瘤、肾癌、肺癌、肝癌、结直肠癌、卵巢癌和乳腺癌等,此细胞治疗具有强大的抗肿瘤活性,刺激机体产生抗肿瘤免疫反应,疗效十分满意,副反应轻微。

  2001.01--2005.08共有274人接受CIK细胞治疗,其中男性198例,女性76例,平均年龄56.34岁。其中消化道(包括食管、胃、肠、胰腺)肿瘤81例、肺癌54例、肝癌36例、肾癌29例、乳腺癌20例、淋巴瘤13例、其他41例。

  接受该治疗患者中,Ⅰ-Ⅱ患者占17.1%,Ⅲ-Ⅳ期患者占82.9%;治疗三个疗程以上者为85例,两个疗程者为48例,其余为一疗程,治疗次数最多为34次。治疗过程中除一过性发热、寒战外未发现其他不良反应,总体有效率达35%。

  2. 树突状细胞(DC)疫苗

  DC是人体内功能最强的专职抗原递呈细胞,具有活化初始型T细胞的功能。大量体内、外实验证实,用不同形式的肿瘤抗原体外冲击致敏DC后应用,可以产生较强的抗肿瘤免疫,并能抑制肿瘤生长和转移。基于DC疫苗的免疫治疗在生物治疗中越来越重要。该中心从1996年开始利用肿瘤细胞溶解物、照射处理的细胞因子修饰的肿瘤细胞、多肽等制备的肿瘤疫苗开展针对肺癌、胃肠道肿瘤、卵巢癌、肾癌进行辅助治疗,结果证实该疗法能有效刺激机体产生抗肿瘤免疫反应,其安全性和可行性已经得到证实,抗肿瘤效果满意,能有效延长术后患者的生存时间,提高患者的生活质量。

  目前,已治疗52例实体肿瘤术后患者,以患者肿瘤标本裂解物作为抗原冲击树突状细胞,其中男性35例,女性17例,平均年龄54.7岁。其中肺癌18例、消化道(包括食管、胃、肠、胰腺)肿瘤12例、肾癌9例、肝癌4例、黑色素瘤4例、淋巴瘤4例、其他1例。接受该治疗患者中,Ⅰ-Ⅱ患者占69.2%,Ⅲ期患者占30.8%;治疗三个疗程以上者为48例,治疗次数最多为14次。治疗过程中除一过性发热、寒战外未发现明显副作用。

  3. GM-CSF修饰肿瘤疫苗

  GM-CSF是一种强烈刺激巨噬细胞和树突细胞等抗原呈递细胞的增殖、分化、活化、成熟和趋化的细胞因子。该治疗方法使用完整“肿瘤细胞”,提供全部包括已知和未知的与患者肿瘤相匹配的抗原,能强力诱导机体抗肿瘤特异性免疫应答。该治疗方法的常规过程如下:手术→收集肿瘤标本→疫苗制备→皮内/皮下注射→临床疗效及免疫反应监测,注射每次间隔1周;共注射6次。目前国内外已应用于多种肿瘤包括:前列腺癌、非小细胞肺癌、卵巢癌、胰腺癌、肾癌、多发性骨髓瘤、急性粒细胞白血病等。生物治疗科应用GM-CSF修饰肿瘤疫苗技术在治疗肺癌、结肠癌、肾癌及黑色素瘤等方面取得了进展。

  本科室用GM-CSF疫苗治疗了数十位肿瘤患者,结果显示抗肿瘤效果显著,能有效延长术后患者的生存时间,提高生活质量。其中用GM-CSF疫苗治疗了9位转移性肺癌患者,其中女性患者4例,男性患者5例,平均年龄57.5岁。5位患者出现DTH阳性(阳性率55.6%)。治疗4-6次后随访发现7人SD (Disease stabilisation ),1人PD(progression stabilisation ),1人死亡。

  4. 亲缘间半相合异基因造血干细胞联合个体化生物治疗的基础与临床研究

  异基因造血干细胞移植可以利用GVT(移植物抗肿瘤)效应实现对化疗抵抗的实体瘤的抑制和杀伤,显著降低移植相关发病率和死亡率。本科室目前利用活化单倍体相合外周造血干细胞免疫治疗方法治疗了上百例晚期化疗耐药性或转移性实体肿瘤,包括肺癌、肾癌、卵巢癌及高危乳腺癌,临床试验显示具有强大的抗肿瘤活性,刺激机体产生有效的抗肿瘤效应,临床效果十分满意。

  生物治疗中心自成立以来,注重临床与科研工作的相辅相成,先后承担国家级以及省部级科研课题多项,获省部级科研成果奖多项,在国内外期刊发表论文数十篇,参与编著及编译专著与译著多部。每年培养硕、博士研究生数十名,积极开展国内和国外间学术交流,将先进技术及理念与我国具体情况相结合,推动了本科的学术发展。

1. CIK cell therapy

2001.01 - 2005.08 A total of 274 people received CIK cell therapy, 198 patients were male, female 76 cases, with an average age of 56.34 years. In which the digestive tract (including esophagus, stomach, intestines, pancreas) tumors of 81 cases, 54 cases of lung cancer, liver cancer, 36 cases of renal cell carcinoma 29 cases, 20 cases of breast cancer, lymphoma in 13 cases, the other 41 cases.

Receiving the treatment of patients, Ⅰ - Ⅱ patients accounted for 17.1%, Ⅲ - Ⅳ period accounted for 82.9% of patients; treatment for more than three courses of 85 cases, two courses were 48 cases, the rest is a course of treatment, number of treatment up to 34. The course of treatment, in addition to a transient fever, chills, and other adverse reactions not found outside, the overall effective rate was 35%.

2. Dendritic cells (DC) vaccines

DC is the human body the most powerful professional antigen-presenting cells, with activation of naive T-cell function. A large number vivo and in vitro experiments confirmed that the different forms of tumor antigen pulsed DC in vitro after the application, can generate strong anti-tumor immunity, and can inhibit tumor growth and metastasis. DC-based vaccine therapy in biological treatment is increasingly important. The Center since 1996, the use of tumor cell lysate, irradiated cytokine-modified tumor cells, peptides such as tumor vaccine prepared to carry out targeted lung cancer, gastrointestinal cancer, ovarian cancer, renal cancer adjuvant therapy, the results confirmed that the therapy can effectively stimulate the body to produce anti-tumor immune response, its safety and feasibility have been confirmed, the anti-tumor effects of satisfaction, can effectively prolong the survival of patients after time and improve patient quality of life.

At present, 52 patients with solid tumors after treatment of patients, patient tumor samples lysates as antigen pulsed dendritic cells, 35 cases were males and 17 females with a mean age of 54.7 years. 3. GM-CSF-modified tumor vaccine

GM-CSF is a strong incentive for macrophages and dendritic cells and other antigen-presenting cell proliferation, differentiation, activation, maturation and chemotactic cytokines. This treatment method uses a complete "tumor cell", to provide full, including known and unknown patient's tumor antigens match, can induce a strong anti-tumor-specific immune response.
